We use "ever" in present perfect questions to ask if someone has done something at any time in their life (until now).
Structure:
Have/Has + subject + ever + past participle?
*We use "Have" with I, you, we, they
We use "Has" with he, she, it*
*"Ever" usually goes before the past participle.
